Buy back setup...help

You actually read it correctly, what you are asking currently isn't a configurable option, but can be done manually (more on that later). The buy back currently is the cost for any life to stay in the pool after missing a pick, you can't buy a life ahead of time. I'm going to improve the 'help' text tonight to hopefully make this more clear.

Here is how you would do this manually.

1) Configure your league for 2 lives, keep the buyback cost set to 0, and leave the last buyback week set to 21.

2) If someone wants an extra life before week 8, they pay you the cost. If you want, you can track this on the  Manager Control Panel -> Manage Player Fees & Prizes page. Just subtract the cost you want to charge in the 'Fees Paid' column for that player. That way they'll see a reminder of what they owe you when they log in. When they pay you can then return to this page and update the 'Fees Paid' column accordingly.

3) When players who paid for the 3rd life run out of lives (after their 2nd loss),  go to Manager Control Panel -> Edit Player Status, and change their status from Out of Lives to Active. This will move them from 0 to one life. 

4) You'll then need to adjust the Buyback field on the Standings page, which in kind will update the Total Pot value. Where do you do this? In a future feature, that I'll have ready for you before you need it. :)
